Tier 1: Fiduciary Monitoring & Maintenance Pathway

Designed for clients who are clinically stabilized but require independent oversight to satisfy court mandates or long-distance family reassurance.

  • Routine Touchpoints: One (1) pre-scheduled clinical home or facility visit per month to evaluate overall well-being, baseline health status, and nutritional security.

  • Legal & Fiduciary Support: Ongoing document tracking to feed into the Annual Guardianship Court Summary Template.

  • Environmental Oversight: One general home safety and local storm shelter accessibility check per quarter.

  • Exclusion Alert: Any additional medical escorting or emergency interventions outside the monthly visit are billed separately at a prorated hourly rate of $150.00/hour.

Tier 2: Coordination & Stabilization Pathway

Our most popular tier. Designed for college transition students needing executive scaffolding, seniors navigating new diagnoses, or adults with developmental disabilities (IDD).

  • Routine Touchpoints: Two (2) in-person visits per month (spaced bi-weekly) to manage care continuity, monitor independent living indicators, or complete campus dorm wellness checks.

  • Clinical & Academic Advocacy: Up to 3 hours per month of direct coordination with Certified Community Behavioral Health Clinics (CCBHCs), specialty medical networks (Saint Francis/Ascension St. John), or university accessibility offices.

  • Medication Safety: Bi-weekly alignment of pill planners or validation of local delivery drop-offs.

  • Family Communication: One (1) dedicated monthly video conference with out-of-state sponsors to review the Post-Visit Clinical Summary Log trends.

Tier 3: High-Acuteness & Crisis Standby Pathway

$1450.00

An intensive, premium clinical concierge tier for unstable medical scenarios, hospice end-of-life care, or serious mental illness (SMI) behavioral tracking.

  • Routine Touchpoints: Four (4) weekly high-touch in-person visits per month to provide deep clinical oversight, immediate behavioral intervention, or intensive executive function training.

  • 24/7 Priority Standby: Direct, unfettered access to our emergency after-hours line with guaranteed immediate mobilization to Tulsa-area emergency rooms or crisis centers (e.g., Parkside, Laureate).

  • Unlimited Advocacy Coordination: Complete management of hospice care interfaces, inpatient discharge planning, court-ordered guardian interventions, and intensive parental/fiduciary updates.

  • Extreme Weather Protocol: Immediate physical execution of emergency power backup checks and evacuation plans ahead of forecasted regional severe weather.
